Well, your English is not very clear but has already answered your own question. The clearance volume is rather small between the top of the piston in full TDC (Top Dead Center) and the butt. The cylinder is the volume swept by the piston, and up and down the cylinder. In the example you give (which is a typical diesel ratio) is 22 times the volume of discharge.
